Googleは2026年3月10日、Imperial College Londonおよび英国NHSと進めてきた breast cancer screening 研究の結果を公開した。今回の発表は、medical AIにとって最も難しい論点の一つである「大規模screening環境でAIが検出性能を高めながら、human specialistが頼る臨床上の安全装置を損なわずに機能できるか」に正面から向き合っている。Googleによれば、Nature Cancerに掲載された2本の研究で、同社の experimental research AI system は従来screeningで見逃されていた interval cancer の25%を特定した。interval cancer は症状が出てから発見されることが多く、治療難度が上がりやすいため重要度が高い。
第1の研究では、125,000人分の mammogram を用いて AI による読影と expert radiologist の読影精度を比較した。Googleによれば、このシステムは見逃されていた interval cancer の25%を拾い上げただけでなく、expert radiologist よりも多くの invasive cancer と総cancer数を検出し、初回scanを受ける女性では false positive も少なかった。screening program は単に検出数だけでなく、不必要な recall や追加検査の負担をどれだけ抑えられるかでも評価されるため、この点は実務上の意味が大きい。
第2の研究は、NHSの double-reading workflow にAIを組み込んだとき、実際に workforce pressure を減らせるかを検証した。英国では2人の specialist が同じ mammogram に同意する必要があり、意見が割れた場合は arbitration panel が最終判断を下す。Googleは、50,000人超の scan を対象にした結果、AIを second reader として使うと screening workload を推計40%削減できると述べた。specialist 1人あたり年間約5,000件の scan を確認する一方、専用の読影時間は限られており、radiologist shortage も続いているため、この数字は非常に重い意味を持つ。
もっとも、Googleは限界も明確に示している。arbitration の場面では、AIが検出した cancer を specialist が覆すケースもあり、trust、workflow設計、human-AI interaction がなお未解決であることが分かった。また、Londonの12の NHS screening site で 9,000件超をリアルタイム処理した observational feasibility study からは、clinical AI は plug-and-play 製品ではなく、病院ごとの workflow、equipment、患者集団に合わせた calibration が必要だと結論づけている。それでも、検出改善、workload削減、そして実運用に近いworkflowデータが同時に示されたことで、今回の発表は今月の medical AI 分野でも特に具体性の高い更新と言える。
